Student Registration 2024-2025

Eden Central School District now has an online express system for registration. In-person appointments are not necessary.

Please fill out the complete form and add all the appropriate documents. Your child is not considered registered until we receive all the appropriate documents and information.  

ONLY  a parent or legal guardian can register a student for enrollment in our district.   
If you are not the parent or legal guardian, you cannot use our online registration process.  You MUST call (716) 992-3636 or email registrar@edencsd.org

The following documents are required at time of registration:
  • Proof of Birth: You must provide a student’s birth certificate or other satisfactory evidence of age.
  • Proof of Immunization (with official stamp or signature of health care provider)
  • Recent Health Physical 
  • Valid Drivers License
  • Proof of Residency (a notarized residency affidavit is required in addition to the following:
    • Recent utility bills
    • Lease (renters only)
    • Closing statement or deed (if living with family or friends)
    • Other acceptable proofs of residency are listed in the directions on the residency affidavit form. 
  • Record Release Affidavit 
  • EdenCSD Health History Form
Special Circumstances
  • For Divorced Parents – please provide appropriate documentation of custody. This includes: 
    • Court Custody Papers
    • Divorce Documents indicating Custody
    • Contact information and mailing addresses for any legal parents who do not reside in the home with the child
  • Foster Parents - please provide a 2999 Form
