Frequently Asked Questions MS/HS

Newly graduated students will receive a copy of their immunization with their diploma in the mail. Please always keep a copy for your records as your future employers and educational programs will require these many times throughout your life. 

School Health records are retained until the student turns age 27. If you are older than 27 years of age, then the school no longer has a record of immunizations. Please contact your doctor’s office. Often times, records can be found in the New York States Immunization Information System (NYSIIS) that your pediatrician's office has access to. 

If you were born after 1995, then please contact our High School secretary at 716-992-3600 and provide your date of birth and a return phone number to request these documents from our records retention. 

Yes, you will need to complete the "Self Medication" forms located on the Eden CSD website by going to "Services' and clicking "Nurse Office". Complete both pages of the "Self Medication" forms for each medication and have a parent and provider sign both pages. 

Please Note: Students cannot transport their medications. A parent must drop off the medications at the beginning of the school year and then pick them up at the end of the school year. The nurse's office does not have any medications. All medications, including over-the counter medications such as Tylenol must be provided by a parent from home and must include a doctor’s order with signed parent permission included in the "Self Medication" form found online.

Yes, if it is a rescue medication such as an inhaler or epi-pen. You will need to complete the self-medication forms located on the Eden CSD website by going to "Services' and clicking "Nurse Office". Complete both pages of the "Self Medication" forms for each medication and have a parent and provider sign both pages. Don't forget to check the box "Independent carry" on the 1st page of the form.  

Please Note: Students cannot carry any controlled prescription medications listed on the DEA Drug schedule. All controlled prescription drugs must remain locked in the nurse's office (i.e. Adderall). 

You can enroll your child for school on the Eden CSD website any time of year by going to "Parents/Students" and clicking on "Student Registration". Enrollment is done through our online service called "Family ID'. Although you can enroll your child, they will not be approved for enrollment until all medical documentation is provided. This includes:

  • New York State Physical School Health Examination Form (within the last 12 months)
  • A Health History Form
  • Record of immunizations. (Must all be up to date prior to approval of enrollment)
  • Health forms can be found on the Eden CSD website by going to "Services' and clicking "Nurse Office".

Yes! Foreign exchange students are required to have a NYS Physical completed prior to enrollment. Health forms can be found on the Eden CSD website by going to "Services'  and clicking "Nurse Office".

Immunization guidelines vary greatly from country to country and is rarely the same as the guidelines set by New York State. Your foreign exchange student can get a head start on determining which vaccines they require by contacting the Erie County Health Department Immunizations Bureau at (716)-858-2371. It does not matter if vaccines are received in the USA or elsewhere, as long as proof of vaccination documentation is provided prior to enrollment
