FOR CURRENT STUDENTS

You can pick up a working paper application at the guidance office anytime during business hours 7:00 am to 3:30 pm.  Fill out Part I only and have a parent sign.  Bring completed form back to the guidance office and we will verify that you have a current physical within the past year then we will issue you the working paper card.

FOR OUT OF DISTRICT STUDENTS

If you attend an out of district private school you can print the form below and bring the completed form, proof of age (birth certificate, drivers license, etc.) and proof of physical within the last year to the high school guidance office to receive your working paper card.
