IMMUNIZATIONS
Requirements -

Beginning September 2007, all students born on or after January 1, 1994 entering 6th grade must provide documentation of having had the booster vaccine for diphtheria, tetanus and pertussis.

All students entering the 6th grade, born on or after January 1st 1994, are required to have the Varicella (Chicken Pox) vaccine or have medical documentation of having had the disease.

All students entering 4th grade in September must have an updated physical exam; either by their own physician or as provided by our school physician.
- All students entering 7th grade on or after 9/1/2000 will be required to show proof of having 3 doses of Hepatitis B vaccine.
This is in addition to the following:
- Diphtheria - 3 doses
- Polio - 3 doses
- Mumps - 1 dose
- Rubella - 1 dose
- Measles - 2 doses
PHYSICALS
Physicals are required by New York State - all students new to the district, as well as those entering Kindergarten, 2nd, 4th, 7th, and 10th grade must have a physical exam done either by their private doctor or the school doctor. Students participating in sports during the school year must have a physical before the sport begins.
MEDICATION
Students required to take medication during the school day must comply with the following guidelines:

A written order from a licensed physician must accompany the medication in the original container.

Parents/guardians are requested to transport the medication to the nurse’s office where it will be kept in a locked cupboard.
- If a student is to carry their own inhaler, etc., the parent and the doctor must complete a form which will be kept in the student’s permanent health record.
